Do I have to have a prescription to order a hormone test?

Canary Club members do not need a prescription to order a test. The Canary Club is an educational advisory group with a team of medical advisors headed by Richard Shames, M.D.

Under CLIA (Clinical Laboratory Improvement Amendments) regulations, testing varies by state and requires a written or electronic request for patient testing (493.1241). The exception to this regulation is those states that allow Direct Access Testing (DAT), which allows consumers to test without a prescription.  Please note that most insurance companies will not cover the cost of the testing unless your medical professional has ordered and provided a written prescription for the testing.

ZRT and Labrix are CLIA certified labs that provides the service of collecting and analyzing specimen samples. You will be submitting health information and ordering tests for informational purposes only.  You are responsible for taking the test to your primary care physician for any additional interpretation and neither ZRT, nor Canary Club, are diagnosing or treating any condition through your use of this service.

Are there specific herbal supplements/vitamins that can affect hormone levels and if so, how long must I avoid taking them prior to collecting my hormone sample?

Please list all hormones, supplements, and vitamins that you are taking on your requisition form. However, while vitamins may improve your symptoms, they do not generally change lab results. Check with your health professional for further information.

The following products are available over-the-counter and may affect lab results:

progesterone, pregnenolone, DHEA, 7-Keto-DHEA, hydrocortisone (cortisol).

Continue to use topical hormones daily as prescribed, but avoid applying them to the face and neck areas for 3 days prior to testing. Rub in topical hormones without using fingers (e.g. wrist to wrist) for at least two days prior to collection. Skip use of anti-aging creams that may contain hormone traces.

NOTE: 7-Keto-DHEA is a variant form of DHEA and can create false elevations of DHEA and testosterone test results. Please discontinue use of sublingual 7-Keto-DHEAS for 3 days prior to testing. If using pill form, follow the oral collection instructions.

Are there specific skin care products (e.g. anti-aging creams and moisturizers) that can affect hormone levels and if so, how long must I avoid applying them prior to collecting my hormone sample?

Read the product label. If the product contains progesterone, pregnenolone, or DHEA, please list them on your requisition form and avoid applying them to the face and neck areas for 3 days prior to testing.

Can I send my hormone sample via regular mail or should it go priority?

A prepaid shipping envelope is included with the test kit for people ordering within the United States. You may send your sample priority mail if you wish to ensure more rapid delivery. For those residing outside of the United States, shipping to you is $35.00 and it is your responsibility to cover the cost of your return shipping. Because these samples are not bio-hazardous, there should not be any restrictions imposed by customs.

Will you bill my insurance carrier?

You  prepay for services to receive the discounted rate and handle your own insurance claim. Upon receipt of payment in full, The Laboratory will provide you with an insurance receipt. You may submit this receipt to your insurance carrier for reimbursement (in accordance with your health plan benefit coverage).

What forms of payment are accepted by the Laboratory?

VISA, Master Card, American Express, Discover Card, personal checks, and US money orders. However, Internet orders must be paid by credit card only. Other arrangements should be made directly with Canary Club.

What if I live outside of the United States?

Prepayment in United States dollars is required. We cannot accept foreign currency or bill insurance companies outside of the United States.
